Primary care membership is NOT traditional health insurance.
Did you know that most primary care providers have between 2,500-3,500 patients? We limit our DPC clinic to under 700 patients total. That means that Chad spends more time with you when you need him. Plus, when he is not actively seeing a patient, he is able to help coordinate your care and research your treatments.
Summit Direct Health does not bill any insurance plans so you won't have a middle-man in your primary care. Decisions about your healthcare will be made by you and Chad without the impersonal management that comes from third-party payers. Instead, patients pay a monthly membership fee which can save money and coverage for excellent treatment. Chad Jarvis, PA-C Chad is a respected and reliable member of the Tooele County medical community. With a Master of Science from ATSU and 20 years of varied medical experience, Chad's expertise provides a sense of assurance to his patients. His multifaceted career spanning Family Practice, Urology, and Orthopedic Surgery, uniquely positions him to understand the connection between physical health and mental wellness. As your DPC physician assistant, Chad's professional journey began with a Bachelor's in Sports Medicine from BYU, where he contributed to the care of the 2002 national champion men's volleyball team. Chad and his wife Jen are parents to six wonderful daughters and 3 son-in-laws! Celebrating nearly 25 years of marriage, they have called Stansbury Park home for two decades. He is committed to providing exceptional direct care to every DPC patient.

Telemedicine, Virtual or In Office visit: $95
Membership fees are charged per calendar month, which starts on the day you sign up. Typically, an appointment costs about $150 per visit so you actually pay less and get more with your Summit Health membership ($95 monthly for as many visits as needed). Unlike the Affordable Care Act plans or traditional insurance, our model focuses on care coordination and primary care services that truly serve patients.
